Registration:

Welcome to the online registration form.  You can fill it out and transmit a copy to the Fall Ball registrar (you'll get a copy via email).  However, be warned!  Until the corresponding funds are received you are not registered.  Date of registration will be the date when monies are actually received. (Translation:  no dough...no deal...no dance.  'nuff said).

Please make out checks payable to BACDS and mail to: Fall Ball, c/o Helena Ivatt, 524B Ocean View Avenue, Santa Cruz, CA 95062 (be sure to get the name and address spelling correct to avoid mail delays)

Be sure to submit one form for each person to be registered.  Also be sure to complete all information to prevent delays.

Extra hands are needed to make this truly a enjoyable event - see our volunteer suggestions.  If you have specifics things you can/can't do, please note them in comments below:

For the potluck buffet I will bring something savory bring something sweet  enclose a donation ($5-$8 suggested)

Advance registration by November 3rd is only $25 per person; after that (and at the door) $27 per person.
