BACDS Fall Ball

Registration:

Welcome to the online registration form.  You can fill it out, print it, and transmit a copy to the Fall Ball registrar.  However, be warned!  Until the corresponding funds are received you are not registered.  Date of registration will be the date when monies are actually received. (Translation:  no dough...no deal...no dance.  'nuff said).

Please make out checks payable to BACDS and mail to: Fall Ball, c/o Helena Ivatt, 1901 Shoreline Drive, Apt 111, Alameda CA 94501-6010 (be sure to get the name and address spelling correct to avoid mail delays)

Be sure to submit one form for each person to be registered.  Also be sure to complete all information to prevent delays.

For the potluck buffet I will bring something savory bring something sweet  enclose a donation ($5-$8 suggested)

Advance registration by November 8th is only $20 per person; after that (and at the door) $25 per person.
